LCS Student Registration forms 2021-2022
Documentation needed for Registration:

For Kindergarten: 2 proofs of address such as copy of a lease or mortgage.
If you are living with someone, you will need to have a Homeowner’s/Renter’s Acknowledgement form filled out and notarized. It is on our website under Registration. Also, we need a copy of a bill with your name going to that address. We can also accept a voter registration card, electric bill, etc.

A copy of the birth certificate, current shot record and current physical form. Your doctors can provide those for you.

For 1st – 5th grade we require two proofs of address.
